The 82.40% top-10-chain ownership-percentage gap is the central caveat (Chen/Grabowski/Konetzka, Health Affairs March 2024).

Not a complete ownership picture. 82.40% of top-10-chain owners and 55.21% of all owners have missing ownership-percentage values. PE/REIT flags are voluntary and incomplete. Independent estimates put true PE ownership at ~13% vs. CMS's ~5% (PESP April 2025).
