Questions

Pricing questions

Can I use this to make decisions about a person?
No. Fonteum screens providers, vendors, and billing entities against supported public exclusion registries for billing eligibility and program-integrity compliance. It is not a consumer report, and its data must not be used for any FCRA permissible purpose — employment, credit, insurance, housing, or professional-licensing eligibility decisions about an individual. See the API terms.
What does the free healthcare screen include?
The first 25 unique ten-digit NPI inputs are screened on demand. The free workflow shows counts and a matched-source breakdown on screen only; it does not create a download, send email, or set up monitoring. A negative may be indeterminate when coverage is stale or incomplete.
Which healthcare registries are checked?
The exclusion result checks OIG LEIE, SAM.gov, and the loaded state Medicaid exclusion lists. It separately reports OIG Corporate Integrity Agreements and CMS civil-money-penalty records, when an NPI-linked record is available. As of August 29, 2026, the state-exclusions serving table represented 13 state Medicaid jurisdictions. Fonteum does not claim national state-list coverage.
What is the $129 signed certificate pack?
A one-off pack for up to 25 NPIs. After checkout, submit the roster in your account; the completed PDF and JSON artifacts are stored there. The pack has an aggregate Ed25519-signed manifest and a signed child attestation for each NPI. The signatures cover the generated artifacts, not the truth of an upstream registry, and the result is not a clearance.
How is the Grounding API metered?
Per lookup — one retrieval with the source and observation metadata available in that response. Inspecting a supplied snapshot id at /verify bills at 0.25 credit. Committed-spend discounts are available at Scale and above.

Integrated, with-data, and fresh-observation counts are separate. No platform-wide source-completeness count is published. Completeness is source-specific and must be evaluated against the named source's expected scope. State coverage is a separate jurisdiction measure.

Source authority is record-specific

Use the issuer named on the record.

Fonteum spans federal, state, and global public publishers. A source page or returned record identifies its issuer and dataset where that metadata is available. A platform registry count does not assign every page to one authority or establish loaded, fresh, or complete coverage.

Reproducible by design

Inspect the evidence each published figure actually supplies.

Source and date

Research pages expose the named public file and observation date where those fields are available. Source-file SHA-256 coverage is separate; facts do not currently link deterministically to signatures.

Available derivation

Studies with a retained release and committed derivation link the SQL or method used. Other studies state the evidence and reproduction limits they actually have.

Daily observations

Dated table row-count observations can detect local drift. They do not imply that an upstream publisher released or Fonteum ingested new data that day.
