Reconciliation Differ: CMS Hospital Records Joined by CCN

Fonteum reconciles federal hospital data by joining CMS Care Compare (5,426 hospitals, quality and identity) with HCRIS cost reports (0 hospitals, financial data) via exact CCN. 0 hospitals (0% of Care Compare) are joinable. Entity resolution links facilities to their NPI in two tiers: 0 deterministic links (confidence 1.000). This page shows 0 deterministic samples with freshness, reconciliation, provenance, and entity-resolution status.

Freshness

When was each source refreshed? CC snapshot date vs. HCRIS fiscal-year end vs. Fonteum ingest date — side by side per hospital.

Reconciliation

How many fields does each source carry alone vs. joined? "CC alone" = quality and identity. "HCRIS alone" = financials. "Joined" = both, keyed by CCN.

Provenance

Which federal dataset backs each displayed cell? This reconciliation view links its displayed source rows to the source authority and dataset URL — not an aggregated score.

Entity Links

CCN → NPI crosswalk, tiered. 0 links are deterministic (confidence 1.000) from CMS Care Compare ASC. Hospitals without an entry show "Unlinked".

Entity graph — canonical entities behind these records

Beyond the CCN→NPI crosswalk above, every identifier resolves to one canonical entity in the entity graph (entity-graph/v1). Counts below are live. Links are tiered: deterministic joins at confidence 1.000, plus probabilistic links each shown with its real matcher confidence. Person, organization, and facility entities and payment-relationship edges are populated from the deterministic backfill; the remaining edge classes (operates / owns / member_of_system / ownership) land with later operator-run lanes and read zero until then. Resolve any identifier via GET /api/v1/entity/resolve?scheme=npi&value=….

Live reconciliation data is temporarily unavailable — the HCRIS cost-report source did not respond for this render. The differ table and CCN match rate are omitted rather than shown with placeholder zeros; they return on the next successful refresh. Methodology, sources, and the entity graph above are unaffected.

Universe
Hospitals present in both the CMS Care Compare Hospital General Information snapshot (2026-05-07) and the CMS Healthcare Cost Report Information System (HCRIS) database, joined by CCN. 0 of 5,426 Care Compare hospitals (0.0%) have an HCRIS cost report.
Sample
0 hospitals drawn from the 0-hospital join universe. Sorted by CCN ascending; every 1th record selected. Deterministic across deploys — same ordering every render.

Limitations

  • Sample is deterministic, not random. Position in the universe is fixed by CCN sort order, so the distribution may over-represent CCN ranges (state prefixes) with dense HCRIS coverage.
  • HCRIS fiscal years vary by hospital (not calendar year). The 'latest_fiscal_year_end' column reflects each hospital's own reporting period.
  • A CCN present in HCRIS but absent from the Care Compare snapshot indicates the hospital has not published quality data via Care Compare in the current release — this is not a Fonteum data gap.
  • No fuzzy or probabilistic matching is performed in the CCN join. Hospitals that appear in both files under slightly different identifiers are not joined.
  • The Entity Links column is tiered: deterministic links (confidence 1.000) come from federal files that carry both CCN and NPI on the same row; probabilistic name/state candidates that have not passed operator review are labeled 'candidate · unreviewed' with their real confidence. Probabilistic links below confidence 0.95 are excluded from this page and the entity API.
  • The entity-graph counts reflect the deterministic backfill: person, organization, and facility entities with exact-identifier links at confidence 1.000, plus payment-relationship edges (edge type 'pays', sourced from CMS Open Payments) at confidence 1.000. The remaining edge classes (operates / owns / member_of_system / ownership) are pending operator-run backfill lanes, so those counts read zero until then. All current links are deterministic; probabilistic links, when they land, will always display their real matcher confidence.

Frequently asked questions

What is a reconciliation differ?
A reconciliation differ compares the same entity — here, a Medicare-certified hospital — across multiple federal data files to show what you gain, and what you miss, when you use each file alone. Fonteum's differ joins CMS Care Compare (quality and identity data) with HCRIS (financial data) by the 6-digit CMS Certification Number, then shows freshness, field coverage, and provenance side by side for each hospital.
How are hospitals sampled in this table?
0 hospitals drawn from the 0-hospital join universe. Sorted by CCN ascending; every 1th record selected. Deterministic across deploys — same ordering every render.
What is the CCN match rate between Care Compare and HCRIS?
0 of 5,426 Care Compare hospitals (0%) have a matching HCRIS cost report when joined by exact CCN. The remaining 5,426 hospitals have quality data in Care Compare but no current HCRIS cost report, typically because they have not yet submitted or been processed by CMS in the current HCRIS release.
Is probabilistic or fuzzy matching used?
The headline CCN join uses only exact 6-digit matches. The entity-resolution crosswalk (column 5) is tiered: 0 deterministic links (confidence 1.000) from CMS Care Compare ASC, which publishes NPI alongside CCN. Each match method's real confidence range is shown in the breakdown above — deterministic links are never blended with probabilistic ones.
What is the entity-resolution column?
The Entity Links column shows the CCN → NPI crosswalk for each hospital, by tier. 0 links are deterministic (confidence 1.000) from CMS Care Compare Ambulatory Surgical Centers. Hospitals without a crosswalk entry are marked "Unlinked".

Fonteum is a public-records evidence platform covering US, UK & Australia public records, plus global sanctions and entity identifiers, spanning healthcare, federal procurement, sanctions and watchlists, federal enforcement, intellectual property, securities filings, and global company registers. Production registry status is reported separately from loaded and fresh coverage. Dated platform counts come from the public platform-stats snapshot; source-file hashes are reported where present, and deterministic fact-to-signature linkage is not claimed.

Integrated, with-data, and fresh-observation counts are separate. No platform-wide source-completeness count is published. Completeness is source-specific and must be evaluated against the named source's expected scope. State coverage is a separate jurisdiction measure.

Source authority is record-specific

Use the issuer named on the record.

Fonteum spans federal, state, and global public publishers. A source page or returned record identifies its issuer and dataset where that metadata is available. A platform registry count does not assign every page to one authority or establish loaded, fresh, or complete coverage.

Reproducible by design

Inspect the evidence each published figure actually supplies.

Source and date

Research pages expose the named public file and observation date where those fields are available. Source-file SHA-256 coverage is separate; facts do not currently link deterministically to signatures.

Available derivation

Studies with a retained release and committed derivation link the SQL or method used. Other studies state the evidence and reproduction limits they actually have.

Daily observations

Dated table row-count observations can detect local drift. They do not imply that an upstream publisher released or Fonteum ingested new data that day.
