Who owns what. Federal source. Chain-complete.

The CMS PECOS SNF All Owners dataset maps every CMS-certified skilled nursing facility to its ownership chain — individual owners, managing employees, and affiliated entities.

ownership records across facilities nationwide, with Affiliated Entity IDs enabling chain-level aggregation without proprietary graph overlays.

Chain size, concentration, and related-entity mapping are derivable from the federal record alone. The /ownership brand-hub surfaces the analysis; the underlying table is queryable via the API with CCN or Affiliated Entity ID.

Federal cost report data. No third-party resell required.

CMS HCRIS (Healthcare Cost Report Information System) contains facility-level financial statements filed annually by Medicare-participating providers. Fonteum's

-row summary layer surfaces operating margin, revenue, Medicare/Medicaid payer mix, and bed count — all derivable from the public cost report without a Definitive Healthcare or Strata subscription.

CCN-keyed — any facility in the ownership graph cross-joins to its cost report history in a single query. Fiscal year end dates vary by provider; the methodology version pins the exact cost report period included.

facilities

The CCN identity backbone for every cross-source join.

The CMS Provider of Services (POS) file is the quarterly canonical facility registry for all CMS-certified providers: SNFs, home health agencies, hospices, dialysis facilities, hospitals, and ASCs.

CCN-keyed facilities. Every other CMS dataset — Care Compare, HCRIS, penalties, deficiencies — joins on CCN. Fonteum maintains POS as the identity backbone so cross-dataset joins are point-and-click rather than a mapping exercise.

Natural-language query examples

Three questions PE diligence teams ask.

The Evidence Query API returns source-cited answers backed by federal records — not a vendor summary. Each answer carries the federal source URL, snapshot date, and methodology version.

Staffing-rule exposure by owner

List SNF owners with ≥3 facilities below CMS staffing rule in Florida.

Cross-joins PBJ staffing records, CMS minimum staffing thresholds, POS facility locations, and the SNF All Owners table. Returns owner entity, CCN list, RN hours/resident day, and delta to threshold — per facility, per quarter.

Financial × quality composite

Cost report operating margin × deficiency count × ownership concentration.

Joins HCRIS facility-level operating margins, NH deficiency citation counts (scope/severity weighted), and SNF ownership chain size. Returns an operator-level composite for portfolio screening.

OIG exclusion exposure at ownership chain level

All owners whose entities employ excluded individuals (OIG federal registry).

Cross-joins OIG LEIE exclusion records (NPI-keyed) to SNF ownership records (CCN-keyed). Surfaces any chain where at least one employed individual has an active federal exclusion — the liability a compliance team at an LP would ask for.

Integrated, with-data, and fresh-observation counts are separate. No platform-wide source-completeness count is published. Completeness is source-specific and must be evaluated against the named source's expected scope. State coverage is a separate jurisdiction measure.

Source authority is record-specific

Use the issuer named on the record.

Fonteum spans federal, state, and global public publishers. A source page or returned record identifies its issuer and dataset where that metadata is available. A platform registry count does not assign every page to one authority or establish loaded, fresh, or complete coverage.

Reproducible by design

Inspect the evidence each published figure actually supplies.

Source and date

Research pages expose the named public file and observation date where those fields are available. Source-file SHA-256 coverage is separate; facts do not currently link deterministically to signatures.

Available derivation

Studies with a retained release and committed derivation link the SQL or method used. Other studies state the evidence and reproduction limits they actually have.

Daily observations

Dated table row-count observations can detect local drift. They do not imply that an upstream publisher released or Fonteum ingested new data that day.
